GENERAL REQUIREMENTS

  1. Original Passport

    • Atleast 6 months valid beyond intended stay

    • Must be signed and have atleast 2 blank pages

  2. Application Form (Printed in A4)

  3. Two (2) 2x2 ID Photo

  4. Original Birth Certificate issued within 1 year

  5. Original Marriage Certificate issued within 1 year (if married)

    • If the birth certificate from Philippine Statistics Office (PSA) is unreadable, or has incomplete information, please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.

    • If the birth certificate is “late registration”, please submit in addition a baptismal certificate and school record (Form 137) from high school or elementary and School Yearbook (if possible).If not available, provide explanation letter.

    • If there is no record in PSA, please submit a “Certificate of Non-Record” from PSA together with a birth certificate from Local Civil Registrar

  6. Daily Schedule in Japan

  7. Original Bank Certificate 

  8. Authorization Letter signed by the applicant (Printed in A4)

  9. Request for Multiple Visa Form (Printed in A4)

    • Applicable to applicant with previous Japan visa and without sponsorship from anyone

    • Applicable to first time applicant with a minimum of 1-Million Pesos in the bank certificate

    • Multiple entry visa is subject to the embassy’s approval

SPECIAL REQUIREMENTS

FOR EMPLOYEES

  • Original Employment Certificate with approve leave of absence (must include applicant’s position, date hired, salary and date of return to work)

  • Copy of Company ID

  • Copy of PRC/IBP ID (if professional)

  • Copy of Income Tax Return (2316)


FOR BUSINESSMAN

  • Copy of Business Registration (DTI or SEC)

  • Copy of Mayor’s Permit

  • Copy of Income Tax Return with evidence of tax payments


FOR STUDENTS

  • Original School Certificate

  • Copy of School ID

  • Original Leave of Absence from school (if travelling while classes are ongoing)

  • Guarantee Letter signed by the sponsor and proof of relationship


FOR RETIRE/SENIOR CITIZEN

  • Copy of Senior Citizen ID 


FOR FOREIGN NATIONALS

  • Copy of Alien Certificate of Registration (ACR)

  • Copy of International Card Residence (ICard)


IF THE COMPANY WILL SPONSOR THE TRIP

  • Guarantee letter from company

  • Original bank certificate and bank statements of the company (last 6 months)

  • Copy of business documents and ITR of the company


IF GUARANTOR WILL SHOULDER PART OR ALL OF THE APPLICANT’S TRAVELE EXPENSES, additional requirements:

  • Guarantee Letter signed by the sponsor

  • Proof of relationship between applicant and sponsor

  • Original Bank Statement and copy of Income Tax Return (ITR) of guarantor

  1. SIZE OF DOCUMENT FOR APPLICATION FORMS SHOULD BE A4 SIZE ONLY. 

  2. APPLICANTS ARE EXEMPTED FROM SUBMITTING (4) AND (5) INDICATED ABOVE IF THEY CAN SUBMIT OLD/VALID PASSPORTS WITH USED JAPAN VISA.

  3. IT IS APPLICANT’S RESPONSIBILITY TO ENSURE THAT HE/SHE MEET THE REQUIREMENTS FOR THE GRANT OF A VISA. SUBMISSION OF ANY SUPPORTING DOCUMENTS NOT LISTED ABOVE IS ENCOURAGED (E.G. APPLICANT’S ECONOMIC OR SOCIAL TIES WITH THE PHILIPPINES, URGENT REASONS FOR VISIT: MEDICAL CERTIFICATE, WEDDING INVITATION, ETC)
